How Does Peptide Therapy Work?

Peptides are short chains of amino acids that act as signaling molecules, giving cells instructions. In a physician-guided protocol, a licensed clinician reviews your health, confirms eligibility through a good-faith medical exam, and prescribes an individualized, monitored plan. Peptide therapy is medical care, not a walk-up wellness product.

Peptides are short chains of amino acids, the same building blocks that make up proteins. Where a protein can be hundreds of amino acids long, a peptide is much shorter, and that smaller size lets it fit precisely into specific receptors on cells.

Your body already makes and uses many peptides every day. Insulin is a peptide. So are many of the hormones that help regulate appetite, metabolism, and tissue repair. Because peptides are naturally part of how cells communicate, researchers have studied lab-made versions to see whether they may support specific processes in the body.

It is important to be clear from the start. Some peptide medications are FDA approved. Many others are investigational and are not FDA approved. We explain that difference in plain terms below.

How peptides signal the body

Peptides work primarily as signaling molecules. A peptide travels to a target cell and binds to a matching receptor, a little like a key fitting a lock. That binding delivers a message, prompting the cell to start, slow, or adjust a particular activity.

Different peptides are studied to support different pathways. The GLP-1 class, for example, is designed to interact with receptors involved in appetite regulation and blood sugar. Other researched peptides have been studied in the context of tissue repair, recovery, or cellular function.

Because a peptide only affects cells carrying the receptor it fits, its activity tends to be targeted rather than broad. This is the biology that peptide protocols are built around. It is not a promise of any specific result, and outcomes vary from person to person.

How a physician-guided protocol works

Peptide therapy at WholeHealth Hydration is physician-guided medical care. You cannot simply buy it and start. A licensed physician or nurse practitioner is involved at every step, and treatment follows a clear clinical sequence.

  • Consultation. You meet with a licensed clinician to discuss your health history, goals, current medications, and any conditions that matter for safety.
  • Good-faith medical exam. Before anything is prescribed, the clinician performs a good-faith exam and eligibility screening. This is a required part of responsible prescribing, not a formality.
  • Individualized prescription. If you are an appropriate candidate, the clinician prescribes a specific peptide and dose chosen for you. There is no one-size-fits-all protocol.
  • Monitoring and follow-up. Your response is tracked over time, and the plan is adjusted, continued, or stopped based on how you do and on clinical judgment.
  • Honest candidacy. If peptide therapy is not right or not safe for you, a good clinician will tell you so. Not everyone is a candidate.

How peptides are administered

Most therapeutic peptides are not well absorbed if swallowed, because digestion breaks them down before they can act. For that reason many peptide protocols use a small subcutaneous injection, a shot into the fatty layer just under the skin, often self-administered at home after your clinician shows you how.

Depending on the specific peptide and the prescriber's plan, other forms such as oral, nasal, or topical preparations may be used. The route, the dose, and the schedule are all decisions your clinician makes as part of your individualized prescription.

Any peptide you use through a physician-guided protocol should come from a licensed source with proper oversight. Products labeled research use only are not intended for human use, and buying peptides outside of medical care carries real safety and quality risks.

The honest regulatory picture

This is the part many websites blur, and we will not. The regulatory status of peptides varies widely, and it matters for your safety.

The GLP-1 class is FDA approved for chronic weight management. Semaglutide, sold as Wegovy, was approved by the FDA in June 2021 as an adjunct to a reduced-calorie diet and increased physical activity. Tirzepatide, sold as Zepbound, was approved by the FDA in November 2023 for the same use. These are approved prescription medicines.

Many other peptides are investigational and are not FDA approved. Examples widely discussed online, such as BPC-157, TB-500, Epitalon, Semax, Selank, and GHK-Cu, do not carry FDA approval for the uses people often ask about. The strongest evidence for many of them is preclinical, and large human trials are limited. Where these are used at all, it is under physician supervision, sometimes through a compounding pharmacy.

The regulatory picture for these investigational peptides continues to evolve. The FDA has been evaluating whether certain peptides, including BPC-157 and TB-500, may be used in pharmacy compounding, and as of mid-2026 they are not on the FDA's approved list of bulk substances for compounding. Recommendations from FDA advisory committees are advisory and non-binding, and none of this equals FDA approval. We follow the current regulatory status and will never imply approval where none exists.

Safety, eligibility, and the GLP-1 example

Every peptide protocol starts with screening because safety comes first. The FDA-approved GLP-1 medications are a useful example of how careful prescribing works, and of why a medical exam is not optional.

GLP-1 medications for weight management are prescribed as an adjunct to diet and exercise, not as a replacement for them, and they require ongoing physician monitoring. They also carry specific contraindications your clinician screens for.

  • Key contraindications. GLP-1 medications like semaglutide and tirzepatide are contraindicated in people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). Both carry a boxed warning about thyroid C-cell tumors seen in rodent studies, where the relevance to humans has not been established.
  • Other cautions. A history of pancreatitis and pregnancy are important reasons a clinician may not prescribe these medications. Your full history is reviewed before anything is prescribed.
  • Common side effects. The most common side effects are gastrointestinal, including nausea and other GI upset. Your clinician will discuss what to expect and what to watch for.
  • Results vary. In clinical trials, participants lost weight on average, but published averages are not a personal promise. Your results depend on your body, your plan, and your effort, and they may differ.
